There are a variety of MUD engines/codebases you can use. Each has it's own pros & cons. I prefer CoffeeMUD as it is very modern, written in Java, and has a nice web interface. It can also import a variety of "area" files from other MUD codebase formats.

I offer CoffeeMUD hosting for $14/month with first month free. Cancel before the end and pay nothing. You can go here for more info: https://PocketMUD.com/HostSplash

The answer is...it depends. Some games some with locations, skills and the like, some are barebones ready for you to build your own. I think a lot of the Diki and CircleMUD, and similar were like that.
